A hospital-based skilled care program, or "Swing Bed" program, is a comprehensive inpatient program for patients who have had an acute medical or surgical event as a result of an illness, injury or disease.

We offer an outcome-focused interdisciplinary approach made up of the patient, family and a healthcare team including physicians, nurses, therapists, dietician, and respiratory therapists as needed to deliver care close to home.

The program is usually short term with the goal to improve and return to a more independent setting, typically home, or to a lower level of care hence decreasing the long term cost to both the patient and/or the payor.

What is the Swing Bed Program all about?

There is no place like home, and yet after a major illness or surgery there are times when a person is unable to return to a home environment. Often a patient's insurance will determine if he or she no longer qualifies to stay in the acute hospital setting. They need a place where they can continue their recovery and make progress towards regaining the highest practicable level of independence - that is where we can be of assistance.

Our program is a transitional level of care that is Medicare certified and will allow patients to continue to receive skilled nursing and rehabilitation services without leaving the hospital. Our spacious private rooms offer a quiet setting to enhance your quick recovery.

Our team of healthcare professionals will come together to deliver treatment plans that are unique to each patients recovery goals after joint replacement, major surgery, major accident, heart attack or strokes. We are also here for those who may need nutritional therapy, infusion therapy, or have wounds that are not healing or need special care. Let us assist you in your health care journey!
